The Care Concierge of New England offers a free service to seniors to help them find Assisted Living and/or Memory Care for themselves and their loved ones. We start with an initial consultation; then, we develop a list of properties which we are happy to tour with you to serve as your in-the-moment advocate and guide.

Assisted Living is a community-style setting where residents can receive supportive care services 24 hours a day, 7 days a week. Memory Care services involve secured, supportive environments where residents with Alzheimer's disease and other dementias may age gracefully and peacefully while receiving more focused guidance and care.
The Care Concierge is proud to have strong relationships with Assisted Living and Memory Care providers in Rhode Island, Connecticut, and Massachusetts, and can help you find the absolute best next step for great quality of life.
For Ongoing Peace of Mind
For many families, the move to assisted living is the next step. Once a loved one has chosen a new home, families may need additional, ongoing support and advocacy as their loved one continues on their aging journey. The Care Concierge is proud to manage Support Services, to give families ongoing peace of mind and ongoing advocacy for their loved one.
The Care Concierge will be your personal liaison to your loved one's care team, working with them to coordinate the best possible care for your family member. The Care Concierge will also join you for facility-led family meetings and separately coordinate regular check-in meetings with your loved one's care team on your behalf. If your loved one winds up needing a hospital stay, the Care Concierge is happy to be part of any and all care planning and discharge conversations, as well.
As your loved ones' needs change and evolve over time, you may find yourself in need of different or additional services. Count on the Care Concierge to help organize and manage your care team as it expands by identifying, vetting, and helping you choose the best resources available.
These resources may include: private duty aides; hoarder clean-up; elder law; financial planning; moving companies, and more.
Nursing Home Navigator
There are times where a loved one's care needs may progress to a point where they require the 24/7 medical care available in a skilled nursing setting. The Care Concierge vets and monitors nursing homes with the same care and attention to detail as with assisted living communities, and when the time comes can help you make this transition. Family Advocacy services can continue at nursing homes for no additional charge.
Please note the Care Concierge does not manage Medicaid applications, but will connect clients with experts who do.
For seniors living in an assisted living community - or seniors still living at home - the Care Concierge can be their personal assistant, doing drop-offs and pick-ups for dry cleaning, library books, movies, and more. The Care Concierge will also go grocery shopping for clients, pick up prescriptions and mail, and more!
For seniors who own their home but spend the winters away, trust the Care Concierge to do regular wellness checks on your house, tend to your plants, and keep you informed so you can enjoy your fun, friends, and family. For assisted living residents who own a pet but may be travelling or out due to a hospital stay, the Care Concierge will ensure the pet is fed, watered, and walked until you are home.

Assisted Living & Memory Care Placement
The Care Concierge provides Assisted Living and Memory Care placement services free of charge to clients. CCNE does have referral agreements with a number of AL & MCAL providers; however; recommendations are based entirely on what is best for the client, whether they are a CCNE partner or not.
Support Services Consultation
The Care Concierge will meet with you for a free initial consultation to determine which Support Services services you need and how often. From there, a quote is developed and clients may opt for paying an hourly rate for different services or a management fee.
